Anterior Porcelain Reconstructive Smile Makeover
Our patient, an active adult in a high visibility career, had a number of functional and cosmetic changes he wanted to overcome a life history of anterior occlusion issues, severe diastemas and almost uncontrollable teeth chipping.
Early Treatment Choices Unavailable
Like millions of other adults, our patient didn't have ready access to treatment opportunities that could have addressed what probably started out as a fairly common occlusal problem and excess tooth spacing problem.
Traditional orthodontic treatment could have been used during certain developmental stages during his childhood.
Our patient had no idea during those early years that he would become employed in a career that had social implications as he had now. Looking great, feeling great and smiling from ear to ear was something he now wanted.
His current cosmetic appearance was causing him to be very aware of his teeth. He simply didn't like being self concious about his smile.
Visibility Redefines Dental Health Goals
Our patient had above average oral health habits. Overall dental function and oral hygiene were quite good. Nonetheless, as our patient's career moved him into social exposure that went way beyond just playing basketball... he was no longer content with the appearance of his anterior teeth.
As shown in the adjacent before photos, our patient had mild flaring of his anterior teeth, along with the untreated multiple diastemas. The severe tooth chipping ocurred as a natural result, stemming from the faulty occlusion.
No Time for Braces
As an active adult, our patient had absolutely no interest in treating his dental needs through conventional orthodontic treatment. Waiting 2-3 or more years to achieve certain cosmetic and orthodontic changes was out of the question.
Acknowledging that standard metal braces is probably the best treatment option... he nonetheless wanted something that was better.... that would produce near immediate results. Having a mouth full of metal, he felt, would create social distractions he simply didn't want.
Customized Instant Orthodontics - Occlusal Normalization
Our patient was interested in the principles of using custom feldspathic porcelain restorative products that could create the orthodontic changes and functional results he wanted... but quickly.
Dr. Bram and the patient studied the data generated by the occlusal analysis achieved. Articulation studies indicated that significant change could be created with specialized veneer products combined with tooth preparations, performed on a tooth by tooth basis, that would decrease the amount (degree) of flaring while simultaneously resizing each tooth to the desired width.
A Diagnostic Wax Up Model was used, enabling the patient and Dr. Bram to "sculpt" the exact appearance he wanted. Tooth sizing, color shading and incisal edge design were all controlled by the patient.
Temporary veneers were created that would later serve as the final model before the actual veneers were created, to eliminate unnecessary surprises (a step that is frequently skipped far too often with some dental practices).
Extra Attention to Detail
The patient wore his temporaries for about a week. Based on self report, everything felt natural and looked natural. Occlusal changes were achieved precisely the way the patient wanted them. Minor cosmetic adjustments were made immediately before the fabrication of the final veneer products.
Our final post operative after photo is presented here, showing the degree of change the patient wanted. He obtained the smile he wanted... with the characteristics he wanted... all without traditional ortho treatment.
